Adam began a career in Emergency Medical Services over 25 years ago. He was enlisted in the US Navy where he served at Naval Hospitals and with the US Marine Corps. He also worked extensively in this area as a Paramedic for Woodburn Ambulance Service and Life Flight Network.
Special Interests: He and his family have enjoyed living in Tualatin for 15 years and they enjoy bicycling, hiking, boating, camping and the many other great things that this region has to offer.

Salvador de la Cruz, MD is a family physician and new physician-owner at Tualatin Valley Primary Care, serving individuals and families throughout the southwest Portland area.
After six years practicing family medicine in Tigard, Dr. de la Cruz is excited to begin this next chapter at Tualatin Valley Primary Care. The smaller, independent practice setting gives him the opportunity to focus on what he values most about medicine: spending time with his patients, listening to their concerns, and building relationships that last for years.
A graduate of Willamette University and Oregon Health & Science University, Dr. de la Cruz completed his Family Medicine residency at Providence Milwaukie and has practiced in the Portland area since 2019. He also serves as a medical director for local hospice agencies. His clinical interests include preventive medicine, chronic disease management and geriatrics, while continuing to care for children, adults and seniors as a family physician.
Dr. de la Cruz believes good primary care starts with understanding the person behind the medical chart. He takes time to learn about each patient’s health, family, background and priorities so that care can be tailored to the individual. Over the years, his patients have described his approach as thorough, knowledgeable, caring and nonjudgmental—and have especially valued feeling heard and supported.
Outside of medicine, Dr. de la Cruz enjoys spending time with his wife and two daughters, photography and filmmaking, sports, and life on his family’s small blueberry farm. He moved to Oregon from the Philippines at age 12 and has called Oregon home ever since. You can also frequently find him supporting women’s sports and cheering on local teams.
At Tualatin Valley Primary Care, Dr. de la Cruz looks forward to continuing to care for families throughout Tigard, Tualatin, Sherwood, Wilsonville and the surrounding communities—with more time, better access, and care that remains personal.

Adam Berens is a board-certified Family Nurse Practitioner who graduated with a Master of Science in Nursing from Gonzaga University in 2021. Building on over twelve years of nursing experience, including ten years as a critical care and rapid response nurse, Adam is excited to begin the next chapter of his career with the team at Tualatin Internal Medicine. Adam is a native Oregonian and resides in Clackamas County with his wife, children, and an active German Shepherd dog. He enjoys listening to music, running, following the Portland Trail Blazers and Gonzaga Bulldogs, amateur astronomy, and woodworking. Adam believes strongly in patient and provider relationships that are built on trust and honest communication, patient safety, and health promotion.

Anita Cintron Rivera is a native Oregonian. She graduated with her bachelors of Biology in 2007 from Oregon State University. She received her Masters of Physician Assistant Studies from Pacific University in 2016. As a recipient of both the Pacific University Rural Health Scholarship and also the Oregon Rural Health Loan Forgiveness Scholarship Program, she spent the first years of her career in pediatrics serving a rural and underserved population.
She is passionate about preventative medicine and helping others achieve optimal health. During her free time, she enjoys spending time with her family, exercise in all forms, water painting, and spending time outdoors.
